After a severe amount of Googling, I’ve come to the conclusion that you have only one option: do 1000 calf raises a day and get ankle liposuction. Beauty is pain. The calf raises will burn, but the lipo is a pretty easy outpatient surgery that’s $8,000 (ballpark).
